Ms SK, a 64 yr old lady with Diabetes, Hypertension and wanting to lose weight.
“I felt tired, and had enough of my high blood pressure and diabetes"

SK had historically always been quite active as she was also employed by the navy. She went through a difficult marriage in her thirties, and developed chronic fatigue. In her mid forties she filed for divorce and was a single mother for some time. During that time she developed significant gastrointestinal symptoms and was diagnosed with IBS. She identified several food sensitivities. Since her late forties she developed more and more joint pain and stiffness, which prevented her from being as active as she would like to be.

She had attended multiple weight loss programs, but none of the usual approach led to long lasting and satisfying weight loss results. Her diet was primarily low fat, with plenty of processed carbohydrates such as pasta and bread, low fat wraps, low fat cereals. She focused on plenty of fruits and vegetables, however all non–organic. She lived independently with her new female partner, and had a busy job.

Her initial medications were Metformin 500 mg, Janumet 50/1000 (for diabetes), Ramipril 10mg (for blood pressure), Rosuvastatin 10 mg (for cholesterol), Salbutamol puffer (for asthma), Seretide puffer (for asthma) Initial Symptoms were feeling short of breath and having bad asthma control. She had muscle pain, joint pains which prevented her from being as active as she would like to be, and had a general feeling of fatigue and sluggish most of the time.

SK was put on a Therapeutic Carbohydrate restriction with daily Carbohydrates at around 30g /day. Her diet was comprised of whole foods and a rich variety of low carb vegetables. The focus was on plenty of healthy organic fats and proteins. She was also put on a time restricted eating plan. She had two meals per day. Her eating window was gradually reduced from 8 hours to 4 hours. Most of the time she had only one meal per day, without the need to snack in between. There was no calorie restriction, yet within 6 months she’d lost 24kg.

Additionally, she took a vitamin D supplement, and increased her time outside. Since April she also used anti-inflammatory supplements such Resveratrol, Curcumin, and Quercetin.

Within 4.5 month now all her medications have been carefully decreased then ceased. First the diabetes medications, and later the blood pressure medications. SK was completely off all her medications by mid-June 2022. SK felt a significant improvement with her joint pain within the first few weeks. This enabled her to be more active again, and she started walking and light jogging again. This also helped her with her mood to become significantly better. She also notices that she did not use the Salbutamol puffer as often. This was the first winter where she was not using any puffers at all.

Mr LA, a 52 yr old man with Diabetes, High Cholesterol, and gout.

"I never wanted to have gout again, and wanted to fix my diabetes and high cholesterol"

He was already very active, and always presented in his cycle gear for his appointments. He was riding every day to work and additionally attended the gym 3 times / week. Though he still couldn’t seem to fix his diabetes, high cholesterol, blood pressure or gout.His diet was heavy in carbohydrates with Bagels for breakfast, sushi or chips, or scones for lunch, and mainly bread or chips with vegetables for dinner. He also consumed Pepsi max and milk shakes regularly. On several days per week his family had Asian take away. He admitted to have cravings for salty chips. He had a supportive and nourishing family life with his wife and children, but had occasional work stressors due to and increased work load. His sleep was good, and he felt refreshed after approximately 7-8 hours. He lived independently with his family, and worked as an IT Consultant in a private company. Despite his active lifestyle he reported feeling fatigued a lot of the time.

His initial medications Metformin 1g twice a day, Simvastatin 40mg (for cholesterol), Allopurinol 100mg (for gout), Ramipril 20mg (for blood pressure).

He was put on a Therapeutic Carbohydrate Restriction at around 25 – 30 g Carbohydrates / day. He ceased processed foods, and replaced it with whole food organic low carbohydrate fruits and vegetables. He quit all soft drinks and milk shakes and replaced them with tea or water. Initially he and his family did not have any take aways, but later had the occasional take away. He had a slow start finding his way into the new lifestyle and needed a lot of assistance, but once he started seriously he was very compliant and enjoyed the changes.

After discussions with the team he agreed to try a time restricted eating pattern of 8 hours eating with two meals, and 16 hours fasting. The eating window gradually closed and he naturally had one meal a day on most work days. On weekends he had two meals in a 6-8 hour window. Later he found it very easy to follow the new lifestyle, and his cravings for slaty chips ceases within the first week. He was also commenced on Vit D supplements.

His diabetic medications were gradually reduced and ceased within the first 4 weeks of lifestyle changes. His blood pressure medications were gradually reduced and then ceased within 5 months. His gout attacks did not return with the new changes. He feels better than ever, with no more episodes of fatigue. He is now medication free.

MR SN, a 35 year old male with badly controlled Type 2 diabetes despite multiple medications

This 35 year old male was diagnosed with Type 2 Diabetes in May 2021 and placed on Xigduo and Metofmrin by their GP. The patient self ceased the Xiguo soon afterwards but continued the Metformin after speaking to us. We advised the patient to get a 24 hour glucose monitor which quickly showed them the right way to eat to maintain good glycaemic control. The patient was open to doing some intermittent fasting so this was implemented 2 days a week. Their diet was changed to a low carb keto diet. As they lost 12kg they felt the desire to walk 30 minutes a day and this was encouraged. In October the Metormin was ceased at the patient's request and they continue to improve their glycaemic control and overall wellbeing, off all medication.

Ms GS, a 73 yr old lady with Diabetes and fatty liver disease.

GS had newly diagnosed diabetes and her Non-Alcoholic Fatty Liver disease, High Blood Pressure, High Cholesterol and Reflux (GORD). Her diet was relatively healthy containing fresh fruits and vegetables, as well as pasta, breads, potatoes and sweet fruits, and occasionally ice creams to satisfy her occasional cravings for sweet. Her meat intake was moderate and was mostly chicken. She had frequently a glass of wine for dinner 3 times / week. She lived with her husband, who seemed to have early signs of dementia, as his memory was starting to fade a bit. This caused frequent stress. She was retired and lived independently with her husband. She enjoyed looking after her grandchildren. Her sleep was good, except for those days when she was frustrated about her husband. But overall the relationship was supportive. Overall she felt tired and fatigue a lot of the time.

Her initial medications were Verapamil 80mg (for blood pressure), Simvastatin 40mg (for cholesterol), Pantoprazole 20mg (for reflux), Metformin 500 mg (for diabetes).

She was put on a Therapeutic Carbohydrate restriction with daily Carbohydrates at around 30g /day. Her diet was comprised of whole foods and a rich variety of low carb vegetables. The focus was on healthy organic foods and fats. She was also put on a time restricted eating plan. She had two meals per day within an 8 hours window. There was no calorie restriction. Additionally she was put on Magnesium supplements.

Within 3 month all her medications have been carefully decreased then totally ceased. Her reflux was gone. She felt much more energetic, and clearer. More focused and awake. She had no trouble following the lifestyle changes supported by her health coach. She reduced her alcohol intake drastically. She tried to get her husband onto the same lifestyle, but he continued to buy himself sweet treats. This caused occasional anger and frustration for the patients. However, over time GS learned to accept the fact that her husband did not want to follow her lifestyle, and she was still able to maintain the new changes in her own life.

Despite being off her cholesterol and diabetes medications her bloods significantly improved.

 

Mr PB, a 71 yr old man with hypertension despite a ‘clean diet’

"I wanted to fix my high blood pressure and be able to stop my medication"

Mr PB’s diet was mainly whole fresh foods, his wife was shopping and cooking daily. He and his wife ate breakfast containing processed foods such as cereals, or toast with occasional fruit juices. He avoided fatty foods, and used vegetable oils to cook with. His foods have been mainly non organic. He was retried and lived independently with his wife.

His initial medications were Ramipril 20mg (for blood pressure) and Escitalopram 10mg (anti-depressant) . His main Symptoms were feeling down and depressed and not being able to sleep well because of thoughts.

We commenced him on an anti-inflammatory diet, which excluded processed breakfast foods and polyunsaturated vegetable oils. He used healthier saturated and mono-unsaturated fats to cook with as well as in his salads. He changed his fruits and vegetables to organic products. He was put on a moderate Carbohydrate Diet at around 100g of carbohydrates per day. As he was following his hunger and satiety cues, he ended up naturally with one meal a day, with the occasional day where he had two meals. We focused on healthy proteins and fats in his meals.

We also worked on relaxation techniques and implemented sleeping hygiene. He was writing a gratitude journal. He actively focused on positive thoughts towards his mother, and good memories from his childhood.

He monitored his blood pressure at home, and we could gradually reduce his blood pressure medications within 3 months. He felt far better and more positive, so we could reduce and eventually cease his antidepressant medication as well. His sleep has improved and reports feeling happy and positive. He has ceased all his medication; his blood pressure is healthy and his blood tests have improved. (His HsCRP went from 5.76 to 1.34, fasting insulin from 10 to 6 and HbA1C from 5.7 to 5.5)

Mr WM, a 64 year old male with Obesity and Diabetes

“Life is soo much easier now, I feel better than I have in years!”

64 year old male with sleep apnoea, Type 2 Diabetes and obesity. He'd started to lose sensation under his feet and was getting chronic fungal infections. He weighed 155kg. He wasn't keen on any more medication and was already on metformin. At his first review he had lost 14 kg and has continued to lose 1 kg a week. His sleep quality improved dramatically and he had more energy. His peripheral neuropathy pain has improved as well. Our team helped him change his diet and added in 2 key supplements as he didn't want any more medication. Blood pressure was normal and his HbA1c dropped from 6.8 to 5.8, without any changes in medication. He was happy to continue on his own as he knew what to do, had achieved the results so was motivated and was going to continue following up with his GP.

72-year-old female with raised HbA1c, on the road to Type 2 diabetes. History of partial thyroid removal.
